GEOsmartcampus, Associazione Scientifica Sanità Digitale e Associazione 3040 have launched ReHealth II the ReGeneration healthcare vertical event through a Startup Competition addressed to Startup, PMI, Aspiring Enterpreneurs to select the best innovative projects dedicated to Healthcare for rising a new Digital Healthcare.

Context

To the present day the SSN (Servizio Sanitario Nazionale) is facinga a complex scenario featured by deep demographic, epidemiological and socio-economic changes. Altogether these factors require to renew the ways of treatement, to be focused on the capability to undertake patients in the long term as well as on the systemic implementation of diagnostic-theraupetic innovative models.

In this context, digital technologies can play the role of optimizers of the Healthcare system, turning it into a more efficient system increasingly centred on the patient. A digital system in which people can take on easy and smart access to their own clinical information and to valid tools in support of customized treatment pathways.

In the case of digital health, indeed, innovation represents not a simple replacement of analog and papery to digital systems, but a great opportunity to better health processes and therefore making the whole sanitary system more efficient.

As regards evolutions in technology, the sanitary sphere during the last decade has got to know a never-seen-before development: innovations range from Digital Pathology -with its innovatory digital slides that offer a powerful remotely-consultation tool and give the chance of further medical opinions on a single analysis- to Location referencing. Georeferentiation allows to visualize on a map

  • patients’ position,
  • the presence of sanitary infrastructures,
  • the high-accuracy geographic representation of diseases incidence.

Further applications of Digital Pathology matched with Georeferentiation systems concern: Cognitive Computing and Artificial Intelligence employed in genomics and cancers treatment; Internet of Things expected to give a substantial incentive to Telemedicine; 3D Printing; Apps and Technologies that ease caregivers daily activities in support of frail patients.

Hereinafter some Guiding Themes in which the employ of the aforementioned technologies can surely favour innovative solutions development meant to make Healthcare systems more efficient.

Guiding themes for Anatomy-Pathological Diagnostics

Deep learning: interpretation of digital microscopic images in support of histocytological diagnostics; Development of an innovative approach to reach an algorithm to spot and distinguish between benign and malignant internal injuries. Compared to earlier attempts, to the present day ineffective, it is required an approach equipped with data correlation and acquisition that avails itself of analytic methods and parametres of new formulation and actually capable to differentiate the various pathological conditions. 

Digital acquisition of tissue-source ex vivo: development of innovative solutions to turn small dimension tissue-source (biopsia) into a digital microscopic imagewithout necessarely passing by Lab procedures of inclusion in paraffin, cut in sections and slide coloring. This application would have large employ to fasten intraoperative investigations and all cases of urgent results for patients’s management.

Digital macroscopic examination: 3D reconstruction of radiologic acquisitions of complex operative samples (eg. surgical expiantation for sarcomas) detected in post-operative session. The system developed will aim to allow the definition of injuries extension and show the best tissue-source sampling thought for microscopic examinations, with special attention to the invasion of peritumor anatomical structures  as prognostic index.

Traceability of diagnostic samples: development of innovative solutions about traceability of histo-cytological examinations allowing sure attribution of medical reports to patients. Compared to existing systems the new approach is required to be efficient in terms of usability, ease of use, handling, timesaving and interrelation with software apps.

Patient’s traceability: development of innovative solutions about patients’ traceability within his/her hospitaller treatment iter and during the monitoring at home that akes into account the FSE updates, laboratory examinations included, radiology, anatomical pathology. With particular referral to cancer patients. Compared to existing systems the new approach is required to be efficient in terms of usability, ease of use matched with AI technologies employ.

Guiding themes for Oncological diseases

Diagnostic protocols for medical Apps. there are different applications of the kind; the target of this section is ease of use of the App, to be intuitive and equipped with operating suggestions, based on an algorithm founded on modern guide-lines and constantly upgradeable. The goal is to provide doctors diagnostic alternatives and therapeutic options based on specific input, from patients’ assessment, to make protocols of medical action internationally standardised in the perspective of a transnational integration of healthcare procedures in order to grant patients equity in access to medical care, optimization of resources and reduction of collateral effects.

Caregivers Apps. an important support for cancer patients and in general for people affected by chronic diseases is the Caregiver figure, -often a relative- a person that takes care of a person in nedd of long term assistance. The caregiver attends the patient, drives him/her to medical visits and treatments, makes sure that he/she takes medicines regularly and organize the examinations schedule. On their shoulders weigh a great burden that is often not shared with others, but becomes a single-person job. The App should build a social web for caregivers that can put them in contact with the goal to optimize together some tasks where possible, get information or suggestions, let patients share experience thay may fibd to have in common to help each other, exchance mutual favours. Moreover the App could be an institutional information tool about new therapeutic opportunities and about the availability of positions for diagnostic exams.

Specific lesions 3D acquisition for therapeutic monitoring. Some patients with locally advanced tumors, are candidates for chemotherapy/radiotherapy which aimes at mass reduction, in the perspective of a forthcoming surgical operation; that’s the case of sarcomas, breast carcinomas and vascolar tumors. The monitoring of therapy effects is based on multiple observations measured through time during the patient follow-up and accompanied by photographs or illustrations. Digital reconstruction of injuries images and its possible association with X-ray data would permit a more accurate monitoring and an assessment built on mensurable benchmarks, with regard to response to treatment.

Data analysis algorithms for genomic panels. In this era of precision-medicine along with the exponential increase of drugs capable to detect therapeutic targets identified by molecular alterations, the oncologist is before different treatment options that have to be chosen according to the genomic analysis of the tumor. Unfortunately at times some therapies don’t get the sought effect or sometimes the tumor recurs, even after an initial phase of improvement. An innovative approach to the theme would integrate genomic data with interrelations of the specific mutated molecule in the cell itself (network medicine) and would compare the same mutational molecular data of the tumor with the patient’s genetic germinal asset. This double approach would allow a real tailor-made offer of treatments and to define what are predisposing genetic conditions.

Wearable devices. On the market there are several sensor devices for vital parametres -also remote-controlled-. Their greatest limit is often teh difficulty to get them properly placed, especially in elderly patients or patients who suffer from impaired motor or cognitive functions. An innovative approach may be the development of a wearable (eg. a garment) with installed sensors for relevant data detection such as blood pressure, temperature, heart and respiratory rate, sweating, capable of collecting and sending data to the healthcare professional and equipped with an alert system to work in case of need.

Specific themes for haematology:

App for treatment adherence and for toxicity monitoring. Therapies for neoplastic haematological illnesses are in continuous development: an App for patients to promptly register possible occurrence of side effects that can swiftly be reported to the doctor for a prompt intervention in case of risk in order to benefit from this data collection.

Computational model development for risk of progression evaluation. Some sow-grade lymphoproliferative diseases lend itself to active medical surveillance or to immediate medical treatments in other cases. Patients therefore need to be monitored through time to control pathological developments. It would be really useful a computational mathematic model prognostic for risks of progression based on the analysis of clinical and lab. parametres or other factors indicators of the pathology aggressivity. This stratification of risk-tests would allow to intervene upfront in cases of progressive diseases.
